At the Kaufman Eye Institute, our goal is to provide the finest eye care services. It is customary to pay for medical care the day it is received. We accept Medicare assignment – if possible please bring a recent copy of your explanation of medical benefits for this calendar year showing that you have met your deductible. Otherwise, you will have to meet it during your visit.
We will research and verify your insurance coverage. Often Medicare and your supplement will pay the entire bill.
We will bill your insurance company for you.
We accept many PPO and HMO insurance plans. Please check with your company and our office if you have any questions about coverage.
We are an authorized provider for many routine vision insurance plans, including Vision Service Plan® (VSP), Vision Care Plan® (VCP), and many others. Please contact your insurance company to inquire if we are a provider for you. Routine vision examinations do not address medical issues.
We are currently enrolled as providers with numerous PPO's/HMO's insurance programs. Should you wish more information regarding our current status of participation with any particular one, please contact our insurance department or your insurance company.
Some insurance companies such as Health Maintenance Organizations (HMO's) and Preferred Provider Organizations (PPO's) require second opinions or pre-authorization for various health services. Many times, members of these plans must obtain a referral from their primary care physician in order to be seen in our clinic. If you are unsure of your coverage requirements, please contact your carrier. Please be sure to obtain any required referrals prior to your appointment.
If you are being referred to our office by another medical doctor or optometric physician, you can be assured your visit will be followed up with a written report or a personal phone call to the referring doctor. They will be kept updated on your medical or surgical treatment and you will be referred back to them at the appropriate time.
